Many veterans and their surviving spouses are entitled to significant financial benefits that can help cover the cost of assisted living, memory care, or in-home care — and most of them never apply. Either they do not know the benefit exists, or the application process feels too complicated.
This guide focuses on the VA Aid & Attendance benefit, which is the most widely applicable and most valuable benefit for veterans who need help with daily care. We also cover other programs that may be available depending on your situation.
The Primary Benefit
Aid & Attendance is an enhanced VA pension benefit that provides monthly payments to eligible veterans and surviving spouses who need help with daily activities or are in a care facility. It is not a loan, not a reimbursement, and not taxable income. It is a monthly benefit paid directly to the veteran or surviving spouse.
Rates are adjusted annually. Actual benefit amounts depend on income and other factors.
Wartime periods include World War II, the Korean War, the Vietnam War, and the Gulf War (August 1990 to present). If you are unsure whether your service qualifies, a VA-accredited benefits counselor can confirm.
How to Apply
The Aid & Attendance application process can take three to six months from submission to approval. Starting early — before a care transition if possible — is strongly recommended.
You will need: DD-214 (discharge papers), marriage certificate (if applying as a surviving spouse), medical evidence of care needs, and financial documentation.

This is the Examination for Housebound Status or Permanent Need for Regular Aid and Attendance form, completed by a physician. It documents the medical basis for the benefit.
VA Form 21P-527EZ (for veterans) or VA Form 21P-534EZ (for surviving spouses). Your accredited counselor will help ensure the application is complete and accurate.
Processing times vary. If approved, benefits are typically paid retroactively to the date of application — so starting early matters.
Additional Programs
A needs-based benefit for wartime veterans with limited income and assets. Aid & Attendance is an enhanced pension rate — you apply for both at the same time.
An alternative to Aid & Attendance for veterans who are substantially confined to their home due to a permanent disability. Cannot be received simultaneously with Aid & Attendance.
Allows eligible veterans to receive care from non-VA providers, including some home health and skilled nursing services, when VA facilities are not accessible or available.
